This form is to submit a request to host an event for Unity of Davis. Please complete all required fields. After completion you will receive a confirmation email. No further communication will take place unless we have questions.

Should you have any questions please contact the office at admin@unityofdavis.org.
Please submit requests at least 4-6 weeks in advance in order to allow for scheduling on the church calendar, graphic creation and promotion. If it is not submitted in this timeframe it may not get adequate promotion. Promotion typically begins 3 weeks in advance.
