This form is to submit a planned event for Unity of Davis to be promoted.

TO SUBMIT THIS FORM
1. Ensure that the event has been discussed with the Events Team
2. Determine the best day/time and location for the event on the church calendar with the Events Team
3. Set all details for the event prior to completing the form. If there are details not yet known, please get those details finalized first.
4. Complete the form and all required fields.

After completion you will receive a confirmation email. No further communication will take place unless we have questions.
